The Aligned Founding Family Program gives a small group of Georgia families ongoing pediatric pharmacist support for 12 months—so you have someone familiar with your child’s medication picture to return to as things change.

We begin by looking at your child’s full medication picture and meeting with you to understand what your family is navigating to help you understand what matters and what comes next.

Bi-Weekly Medication Check-ins
You’ll receive ongoing pharmacist-led support throughout the year for medication changes, specialist visits, hospital or emergency department transitions, new concerns, and other meaningful updates.

Quarterly School Check-ins
Your child’s medication routine may look different during school, summer programs, camps, or seasonal schedule changes. Quarterly support gives us dedicated time to review medication with staff.

We work alongside your child’s existing pediatrician, specialists, dispensing pharmacist, and other treating professionals.
We do not replace your child’s healthcare team.
We help you better understand the medication picture and feel more prepared to participate in it.

Founding Family renewal is not automatic. Eligible families may be invited to renew annually at their founding rate based on continued participation and mutual agreement with Aligned.
Frequently Asked Questions

The Founding Family Plan is available to qualifying Georgia families within Aligned’s usual pediatric service scope, with priority given to families managing chronic conditions, medical complexity, multiple medications, or significant medication-management needs.

We are opening the program to five Founding Families.

Founding benefits may extend to eligible children within the same household when they fall within Aligned’s normal age and service scope.

Founding Families may have the opportunity to renew annually at their founding rate. Renewal is not automatic and requires continued participation as well as agreement from both the family and Aligned.
If a family leaves the program and later returns, current standard pricing will apply.
